Compassionate End-of-Life Care Specialist

We are seeking a dedicated and compassionate End-of-Life Care Specialist to join our interdisciplinary team as a Hospice Licensed Vocational Nurse (LVN). As a member of our team, you will make a meaningful difference in the lives of those in need, providing comfort, dignity, and support to individuals facing terminal illness.

About the Role

As an End-of-Life Care Specialist, you will:

  • Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team to carry out the patient's plan of care
  • Perform nursing services planned, delegated, and supervised by the registered nurse

In this role, you will take a holistic approach to care that engages the patient's mind, body, spirit, and family, assessing and providing patient and family/caregiver education and information pertinent to diagnosis and plan of care.

Requirements

To be considered for this role, you must:

  • Have a graduate degree from an accredited vocational nursing program and active LVN license in California
  • Hold one year of hospice LVN experience
  • Possess CPR certification
  • Hold a valid California driver's license, auto insurance, and reliable transportation
  • Be knowledgeable about state and federal home health and hospice regulations
  • Demonstrate clinical proficiency in nursing, strong communication skills, and compassion
