Independent Optometrist

We are seeking an Independent Optometrist to lease space inside our Walmart stores. As a key member of our team, you will provide exceptional patient care and take a leading role in eye and vision care, as well as general health and well-being.

• Establish your own days of practice and hours of operation
• Control your patient schedule and set your fee schedule
• Keep 100% of exam/professional fees

• Equipment/instrumentation provided for a fair market value
• Administrative and/or Opt-Tech support services available (if state allows)

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
