Responsibilities:
  • Provide medical services to patients, including administering medications and educating patients and families on treatment plans.
  • Develop and implement treatment plans in collaboration with the multidisciplinary team.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Monitor patients' progress, identify areas for improvement, and make recommendations for adjustments to treatment plans.
  • Document patient information accurately and thoroughly in electronic medical records.
  • Participate in quality assurance and compliance activities to ensure adherence to regulatory requirements and company policies.
Requirements:
  • Current, unencumbered LPN license in the state where services are rendered.
  • At least two years of experience as an LPN, with a preference for experience in inpatient or residential settings.
  • Knowledge of computer and electronic medical records systems.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
