Responsibilities:- Provide medical services to patients, including administering medications and educating patients and families on treatment plans.
- Develop and implement treatment plans in collaboration with the multidisciplinary team.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Monitor patients' progress, identify areas for improvement, and make recommendations for adjustments to treatment plans.
- Document patient information accurately and thoroughly in electronic medical records.
- Participate in quality assurance and compliance activities to ensure adherence to regulatory requirements and company policies.
- Current, unencumbered LPN license in the state where services are rendered.
- At least two years of experience as an LPN, with a preference for experience in inpatient or residential settings.
- Knowledge of computer and electronic medical records systems.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Experience working in behavioral healthcare or substance use disorder treatment.
- Knowledge of HIPAA regulations and confidentiality requirements.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- A competitive hourly rate of $26-29.
