Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at PACE of Southwest Michigan. As a leading provider of geriatric care, we are dedicated to delivering exceptional services to our participants and their families.
Key Responsibilities:- Participate in initial assessments, care planning, and periodic re-assessments of participants' cases.
- Communicate participant updates to the Interdisciplinary Team and appropriate parties.
- Document updates in the participant's medical record, including assessments and behavior modification plans.
- Coordinate admissions and discharges from facilities deemed appropriate by the IDT.
- Refer participants and families to community agencies or facilities as needed.
- Act as liaison with various organizations and advocate for participants' needs.
- Master's degree in Social Work.
- Two years' experience in a health-related industry preferred.
- Licensed MSW in the State of Michigan.
- Valid Driver's License with a vehicle and safe driving record.
- CPR certification upon hire.
