PACE of Southwest Michigan: A Leader in Geriatric Care

We are seeking a highly skilled and compassionate Case Manager to join our team at PACE of Southwest Michigan. As a leading provider of geriatric care, we are dedicated to delivering exceptional services to our participants and their families.

Key Responsibilities:
  • Participate in initial assessments, care planning, and periodic re-assessments of participants' cases.
  • Communicate participant updates to the Interdisciplinary Team and appropriate parties.
  • Document updates in the participant's medical record, including assessments and behavior modification plans.
  • Coordinate admissions and discharges from facilities deemed appropriate by the IDT.
  • Refer participants and families to community agencies or facilities as needed.
  • Act as liaison with various organizations and advocate for participants' needs.
Requirements:
  • Master's degree in Social Work.
  • Two years' experience in a health-related industry preferred.
  • Licensed MSW in the State of Michigan.
  • Valid Driver's License with a vehicle and safe driving record.
  • CPR certification upon hire.
