Our rehabilitation programs offer ongoing care and specialized treatment to patients throughout their recovery journey, tailored to their individual needs.
Key Responsibilities- Administer medications accurately and safely, considering the patient's age and clinical condition.
- Perform patient care, prioritizing the standard of care for geriatric and general inpatient rehabilitation patients.
- Assess and reassess patients under the supervision of the R.N., identifying and initiating appropriate rehabilitation nursing interventions.
- Conduct timely and accurate QI assessments, updating the interdisciplinary care plan as needed.
- Develop and implement a plan of care, evaluating its effectiveness and updating it based on changes in the patient's condition and individual needs.
- Communicate effectively with patients, families, and healthcare teams, involving them in the formation of the plan of care.
- Manage pain effectively, educating patients and families on pain management techniques.
- Perform treatments and provide services according to level of licensure, maintaining a safe environment for patients, visitors, and staff.
- Participate in Continuous Quality and Performance Improvement (CQPI) activities, offering suggestions for improving patient care, safety, and education.
- Graduate of an accredited licensed practical/vocational nurse program.
- Current state licensure as a Licensed Practical/Vocational Nurse.
- CPR/BCLS certification, with ACLS preferred.
- IV Certified, with certification obtained within 6 months highly preferred.
- Minimum six months' Medical/Surgical experience in an acute care setting, with excellent oral and written communication and interpersonal skills.
