Job Title: Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Rocky River Healthcare of West Park. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Job Summary:

The primary purpose of this position is to promote the physical and mental health of residents by applying the nursing processes of assessment, planning, intervention, implementation, and evaluation. You will work collaboratively with the healthcare team to provide direct nursing care, coordinate medical services, and ensure that the medical and health care needs of residents are met.

Responsibilities:
  • Assess and evaluate the health status of residents, develop and implement individualized care plans, and provide direct nursing care to meet their physical and emotional needs.
  • Coordinate medical services, including psychiatric and medication services, in accordance with regulatory agencies and individual resident needs.
  • Participate in the admission process, clarify initial or hospital discharge medication orders, and contact the Psychiatrist to review the resident's status and medication plan.
  • Develop and implement plans to ensure that the medical and health care needs of residents are met, including coordinating annual physical examinations, ensuring appropriate medical follow-up, and providing special instructions to residents and home staff.
  • Coordinate and review lab work, care with clinics, outside physicians, and hospitals, and provide health teaching to residents and their families.
  • Make independent decisions when circumstances warrant such action and remain calm during emergency situations.
  • Function as part of an interdisciplinary team, attending medication rounds and team meetings, and completing the medical section of the Individual Plan of Service (IPS) that addresses health and illness needs and issues.
  • Deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Be a supportive team member, contribute to, and be an example of team work and team concept.
  • Follow oral and written instructions, be knowledgeable of nursing and medical practices and procedures, and possess leadership and supervisory ability.
  • Participate in the training and orientation of new staff, facility training activities, and community and other public education regarding mission and philosophy.
  • Plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures to provide quality care.
  • Display patience, tact, cheerful disposition, and enthusiasm, and be willing to handle residents based on whatever maturity level they are currently functioning.
  • Seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Not pose a direct threat to the health or safety of other individuals in the workplace.
  • Complete all other tasks as assigned by facility Administrator, Director of Nursing, or designee.
  • Read, write, speak, and understand the English Language.
Qualifications:

Previous experience in a patient care setting, knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care. Two (2) years experience working as a Registered Nurse.

Education and/or Experiences:

Must possess, as a minimum, a nursing degree from an accredited school of nursing, and have valid registration as a registered nurse in this state.

Working Conditions:
  • Works in office areas as well as throughout the facility, and in resident rooms.
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, etc.
  • Communications with the medical staff, nursing services, and other department directors.
  • Attends and participates in continuing education programs.
  • Works beyond normal duty hours, on weekends and holidays, and in other positions temporarily, when necessary.
  • Is subject to call back during emergency conditions (severe weather, evacuation, post-disaster, etc.).
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminates.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
Physical & Sensory Requirements:
  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English Language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people in the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.
