We are seeking a skilled Geriatric Primary Care Nurse Practitioner to join our team at Symbii Provider Services.


As a Nurse Practitioner with Symbii, you will provide high-quality primary care to our geriatric and disabled populations in assisted living and in their homes.


This flexible position comes with a full-time medical assistant and is instrumental in providing competent, cost-effective, timely, and high-quality health care services.


You will be responsible for delivering clinical and associated operational responsibilities, maintaining performance and productivity standards, and providing care for patients from initial history taking, clinical assessment, diagnosis, prescribing medications and treatments, providing education and evaluation of care.


Maintaining compliance with established standards of practice of medicine for Nurse Practitioner in the State of Idaho is also essential.


You will work collaboratively with our team members and community partners, representing Symbii Home Health and Hospice in a professional manner and striving to promote a cooperative atmosphere.


Maintaining professionalism at all times by being dependable, autonomous, and responsible is crucial.


Additionally, you will prioritize workload and utilize work time properly and productively, completing hospice face-to-face visit requirements and assessing hospice eligibility.


This role is ideal for a Nurse Practitioner who is passionate about providing primary care to vulnerable populations and is committed to delivering high-quality patient care.

Job Requirements:


• Master's Degree from an accredited NP program, possess appropriate state licensure, prescriptive authorization, APN authorization, and meet credentialing requirements.


• Must be able to pass a background check and drug test.


• Must maintain a current and valid driver's license and auto insurance and have access to reliable transportation.


• Must be able to read, write, and speak English fluently.


• Ability to communicate effectively, in verbal and written form, with medical partners at various levels, patients, family members, physicians, and representatives of the community.


• Maintain current CPR certification.


$80,000 - $120,000 per year, depending on experience
