Requirements:

To be considered for this position, you must possess:

  • ">
  • A Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school.

    ">
  • Completion of a family medicine residency program.

    ">
  • Active licensure to practice medicine in the state of Indiana.

    ">
  • Certification by the American Board of Family Medicine or eligibility for certification.

    ">
  • CPR/BLS certification.
