Key Responsibilities and QualificationsAs a Clinical Pharmacist, you will be responsible for ensuring the safe, effective, and evidence-based use of medications for our patients. Your advanced knowledge and professional judgment will be essential in all facets of your role. You will act as a patient advocate while collaborating with a multidisciplinary team of healthcare professionals.
- Prepare and dispense medication orders as per physician directives, adhering to established protocols.
- Verify computerized physician orders for accuracy and safety, intervening when necessary.
- Interpret and accurately transcribe medication orders into computerized patient profiles.
- Prepare and dispense various medications, including sterile and chemotherapy preparations, following appropriate safety guidelines.
- Evaluate Total Parenteral Nutrition (TPN) for suitability based on established protocols.
- Manage controlled substances in compliance with legal and corporate regulations.
- Ensure evidence-based medication therapies are provided according to established standards.
- Monitor medication regimens for potential interactions and contraindications.
Required Qualifications:
- B.S. or Pharm D. degree in Pharmacy - Required.
- Current Pharmacist license in good standing with the State Board of Pharmacy - Required.
- Completion of an accredited residency is preferred.
- Two years of hospital pharmacy experience is preferred.
