We are currently seeking a skilled Nurse Practitioner to join our team as a Home Care Clinician in Florida and the US Virgin Islands. Salary: $40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program (estimated salary range: $120,000 - $160,000 per year)
This position requires travel across the entire state of Florida, approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives.
The ideal candidate will have excellent clinical skills, strong communication abilities, and a passion for delivering high-quality patient care in a home setting. As a Home Care Clinician, you will conduct in-home assessments, perform evidence-based practice screenings, and formulate treatment plans to address the complex health needs of our patients.
Key Responsibilities:
- Conduct in-home assessments, including evaluations of past medical history, reviews of systems, medication reconciliation, vital signs, and comprehensive physical exams
- Perform evidence-based practice screenings, including point-of-care testing as appropriate
- Identify diagnoses and develop treatment plans to address the health needs of our patients
- Communicate findings and recommendations to primary care physicians and other healthcare providers
- Provide education and support to patients on disease management, medication adherence, and healthy lifestyle choices
Requirements:
- Current licensure in Florida and the U.S. Virgin Islands (dual traveler role)
- Active NP license or ability to obtain by start date
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and performing physical assessments
Preferred Qualifications:
- 1+ years of clinical experience in a gerontology, cardiology, internal medicine, or endocrinology setting
- Experience in home health care or home visit settings
