Key Responsibilities
  • Implement and monitor patient care plans, ensuring timely and effective delivery of care.
  • Monitor, record, and communicate patient condition as appropriate, maintaining accurate and detailed records.
  • Serve as a primary coordinator of all disciplines, ensuring well-coordinated patient care and seamless communication among team members.
  • Notes and carries out physician and nursing orders, prioritizing tasks and managing time effectively.
  • Assesses and coordinates patient's discharge planning needs, collaborating with the healthcare team to ensure a smooth transition.
  • May round with physicians in an inpatient setting, providing updates and insights to inform care decisions.
  • Applies evidence-based practice and scientific knowledge to nursing practice, tailoring care to meet the unique needs of each patient.
  • Adheres to the SSM Health professional practice model, upholding the highest standards of nursing excellence.
  • Works in a constant state of alertness and safe manner, prioritizing patient and staff safety.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • No experience required, but a strong desire to learn and grow in a dynamic healthcare environment.
Physical Requirements
  • Constant use of speech to share information through oral communication.
  • Constant standing and walking, with frequent sitting, reaching, and keyboard use/data entry.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs, with occasional lifting/moving of patients.
  • Frequent use of vision and depth perception for distances near and far, and to identify and distinguish colors.
  • Frequent use of smell to detect/recognize odors, and hearing to receive oral communication and distinguish body sounds.
Professional Licenses and Certifications
  • Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA).
  • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), or equivalent licensure in other states.
