Job Summary

We're seeking a Patient Services Coordinator to join our team. This role involves scheduling patients for branch field clinicians, communicating with various stakeholders, and managing on-call notebooks and hospitalization logs to enhance continuity of care.

Key Responsibilities:

  • Schedule patients to branch field clinicians
  • Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers
  • Manage on-call notebooks and hospitalization logs

Requirements

  • Must possess a high school diploma or equivalent
  • Must be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services
  • Must have basic demonstrated technology skills, including operation of a mobile device

Estimated Salary: $55,000 - $65,000 per year
