Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in accordance with applicable laws and regulations.
- Develop and implement individualized plans of care, in collaboration with other healthcare professionals, to ensure continuity of patient care.
- Conduct regular evaluations and assessments of patient needs, and make necessary revisions to the plan of care.
- Ensure patients meet home health eligibility and medical necessity guidelines, as defined by payer sources.
- Collaborate with physicians and other healthcare professionals to initiate, develop, and implement plans of care.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
