You will work closely with our interdisciplinary team to develop and implement effective care plans, prioritize patient needs, and ensure seamless communication with physicians and other healthcare professionals.
Key Responsibilities- Utilize the nursing process as a problem-solving model to anticipate and plan for patient and family needs.
- Conduct thorough nursing assessments to identify subtle changes in clinical situations and adjust care plans accordingly.
- Develop and maintain a therapeutic nurse-patient relationship, respecting patient rights and autonomy.
- Communicate effectively using SBAR in all hand-off situations and document patient care accurately.
- Collaborate with Care Managers to coordinate interdisciplinary discharge planning and ensure smooth transitions of care.
- Participate in quality and performance improvement initiatives, contributing to the hospital's strategic plan and goals.
- Associate Degree in Nursing or equivalent required; BSN preferred.
- Minimum one year of clinical nursing experience in Critical Care, with a focus on regulatory guidelines and standard of practice.
- Current California Nursing license and certification in Critical Care Nursing preferred.
- Bilingual skills in Spanish preferred, along with basic computer skills and current BLS and ACLS certifications.
