Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Georgia Home Health, a part of LHC Group. As a LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Responsibilities

As a LPN, you will be responsible for providing technical skilled care to patients in their homes. This includes:
  • Providing services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Completing progress notes for each visit and submitting them to the agency daily.
  • Observing and reporting the patient's nursing needs, instructing the patient's family in the care of the patient, and maintaining a healthy environment for the patient.
  • Observing, recording, and reporting to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.

Requirements

To be successful in this role, you will need:
  • A current LPN licensure in the state of practice.
  • A current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification.
