Key Responsibilities:- Establish and maintain positive relationships with patients and their families, providing education and support throughout the infusion process.
- Assess patient appropriateness for home infusion therapy, ensuring a safe and effective treatment plan.
- Document patient medical history, medication profiles, and treatment plans, ensuring accurate and timely communication with the care team.
- Collaborate with the multidisciplinary care team to develop and implement comprehensive care plans, addressing the unique needs of each patient.
- Administer medications and perform therapy-specific clinical assessments, ensuring the highest level of patient care.
- Communicate patient responses and overall physical assessment with the care team, facilitating timely and effective decision-making.
- Active and unrestricted licensure as a Registered Nurse (RN) in the state(s) of practice.
- Current Basic Life Support (BLS) and/or CPR certification, or willingness to obtain prior to starting employment.
- 1+ years of experience working as an RN in a clinical care setting, with proficiency in infusion therapy.
- 1+ years of pediatric experience, with a strong understanding of pediatric care and treatment.
- Proficient with electronic medical record documentation, with the ability to navigate complex systems.
- Demonstrated ability to work collaboratively with an interdisciplinary team, ensuring seamless communication and care coordination.
- Experience administering infusion therapy in a home or alternate-site setting.
- Bilingual with Spanish, with the ability to communicate effectively with diverse patient populations.
- Knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs.
- Knowledge of the Infusion Nursing Society (INS) standards of practice, policies, and procedures.
