Key Responsibilities:- Conduct ongoing outreach and engagement with members to identify their needs and develop personalized care plans.
- Collaborate with members, providers, and community organizations to address the full continuum of healthcare and social determinant needs.
- Utilize negotiation and influencing skills to secure necessary services and promote lifestyle changes that promote optimal health.
- Document care management processes in compliance with regulatory and accreditation guidelines.
- 2 years' LPN experience or equivalent in behavioral or physical health, social services, or a related field.
- Ability to travel up to 75% within a designated geographic area for in-person case management activities.
- Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint.
- Excellent analytical and problem-solving skills, as well as effective communication and interpersonal skills.
- 2+ years of experience working in the community, visiting members' homes, and working virtually using collaboration tools.
- Case management/managed care experience is preferred.
