Key Responsibilities:
  • Serve as a clinical and emotional resource for the healthcare team, providing guidance for complex patient cases to ensure the highest quality of clinical outcomes.
  • Conduct comprehensive physical and psycho-social assessments, identifying potential barriers to health, managing patient medication profiles, and effectively communicating lab and test results to enhance the patient experience.
  • Assist with triage, manage in-basket messages, conduct annual wellness visits, and coordinate care for chronic conditions to support the healthcare team and ensure continuity of care.
  • Deliver patient education, including reviewing care plans, ensuring patients have a clear understanding of their treatment protocols.
  • Maintain accurate records of medical and nursing interventions through proper documentation and utilization of electronic medical records, ensuring compliance and utility.
  • Identify gaps in care and contribute to the enhancement of the team's quality metrics to maintain an efficient workflow.
  • Uphold patient safety and experience by adhering to all emergency clinical policies and protocols, ensuring an optimal care experience.
