Job Summary

Topanga Terrace is seeking a skilled Licensed Vocational Nurse to join our team in a subacute care setting. As a clinical professional, you will be responsible for coordinating and evaluating the delivery of nursing care and services to our residents. Your expertise will be essential in managing care plans and supervising resident care activities, ensuring compliance with state and federal regulations and facility policies.

Key Responsibilities:

  • Direct day-to-day resident care activities, ensuring the highest level of care and comfort for our residents.
  • Supervise Certified Nursing Assistants in the delivery of care, ensuring proper implementation of the standard of care.
  • Complete medical forms, reports, evaluations, and charting as needed, maintaining accurate and up-to-date records.
  • Participate in the facility's quality assurance program, identifying areas for improvement and implementing changes as needed.
  • Ensure that reference materials, including regulations and policies, are maintained and organized at the nurses' station.
  • Admit, transfer, and discharge residents, ensuring a smooth transition of care.
  • Review and complete incident/accident reports, conducting investigations as required.
  • Interpret facility policies and procedures to staff, residents, visitors, and government agencies as needed.
  • Attend various facility meetings, including care conferences, stand-up meetings, and nurses' meetings, as required.
  • Receive telephone orders from physicians, process them in accordance with facility procedures, and update the care plan as required.
  • Report all care ordered but not delivered, medication errors, charting errors, diet changes, and other discrepancies to the Director of Nursing.
  • Complete medication and treatment passes in a timely manner, ensuring compliance with state and federal regulations and facility policy.
  • Periodically review medication and treatment administration records for completeness and accuracy, reporting any discrepancies to the Director of Nursing.
  • Inform nursing staff of new admissions, including expected time of arrival and room assignment.
  • Make daily resident rounds to observe and evaluate the resident's physical and emotional status.
  • Meet with residents and/or family as needed, providing support and guidance.
  • Notify attending physician, family, and members of the interdisciplinary team when a resident is involved in an incident or accident, or if there is a change in condition.
  • Consult with attending physicians as needed for treatments, medications, and other care-related decisions.
  • Participate in the facility infection control program, monitoring communicable and/or infectious diseases among residents and staff.
  • Participate in procedures for reporting hazardous conditions or equipment.
  • Partner with the Director of Nursing to ensure staff coverage requirements are met.
  • Give and take report upon beginning and ending of shift.
  • Other duties as assigned.
