  • Developing and implementing individualized nutrition plans
  • Providing nutrition education to participants and caregivers
  • Collaborating with the interdisciplinary team to ensure whole-person care

Responsibilities

  • Assessing participant's access to food and making referrals as needed
  • Determining participants' need for liquid nutritional supplements and ordering accordingly
  • Monitoring food intake and communicating information with the care team

Requirements

  • Bachelor's degree in food science, nutrition, or dietetics
  • Current medical terminology and ability to complete written assessments and document participant care
  • Certification by CDR as a registered dietitian
  • A minimum of one year experience working with the frail or elderly
