Job Summary
We are seeking a skilled and compassionate Licensed Practical Nurse to join our Medical-Surgical Unit team. As an LPN, you will provide high-quality patient care, working collaboratively with our interdisciplinary team to deliver safe and effective care. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education to patients and families.
Responsibilities
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Assess patient needs and develop individualized care plans to meet those needs.
- Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
- Provide education to patients and families on disease management, medication use, and other health-related topics.
Requirements
To be successful in this role, you will need to have a current license to practice as a Licensed Practical Nurse in the State of Vermont, as well as a strong foundation in nursing principles and practices. You will also need to be able to work effectively in a fast-paced environment, prioritize tasks, and maintain accurate records.
