Job Summary

We are seeking dedicated and compassionate healthcare professionals to join our home care team serving the Merrimack Valley and North Shore areas of Massachusetts.

Responsibilities
  • Conduct thorough assessments of patients' physical abilities, medical histories, and home environments to develop personalized treatment plans.
  • Provide skilled physical therapy services including therapeutic exercises, manual therapy techniques, gait training, and balance training.
  • Educate patients and their caregivers on proper techniques for exercises and activities to promote recovery and maintain functional independence within their home environment.
  • Monitor and document patients' progress, modifying treatment plans as necessary to achieve therapeutic goals and ensure patient safety.
  • Collaborate closely with physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
  • Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
  • Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.
Requirements
  • Degree in Physical Therapy or Associate's degree from an accredited Physical Therapist Assistant program.
  • Current state licensure or eligibility for licensure as a Physical Therapist or Physical Therapist Assistant in Massachusetts.
  • CPR certification.
  • Must pass a background check.
  • Clinical experience in a healthcare setting, with preference given to those with experience in home care or community-based settings.
  • Strong interpersonal skills, with the ability to communicate effectively with patients, caregivers, and healthcare team members.
  • Ability to work independently and manage time effectively in a home care environment.
  • Compassionate and patient-centered approach to care, demonstrating sensitivity to cultural diversity and the unique needs of the Merrimack Valley and North Shore residents.
Working Conditions

Provides care in patients' homes within the Merrimack Valley and North Shore areas, with travel between appointments required.

Flexible scheduling allows you to choose your own hours, typically Monday to Friday, to accommodate patients' needs.

Must have reliable transportation and be able to navigate local communities efficiently.

Compensation

PT per diem rate:

Evaluations: $65 per evaluation

Visits: $55 per visit

PTA per diem rate:

Visits: $45 per visit
