We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CBS Lexington KY NRL. As a LPN Home Care specialist, you will provide high-quality patient care and perform technical skilled care in the patient's home.
Key Responsibilities:- Provide quality patient care and perform technical skilled care in the patient's home
- Develop and implement individualized care plans in collaboration with the healthcare team
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
- Administer medications and treatments as prescribed by the physician
- Provide education and support to patients and their families on disease management and self-care
- Current LPN licensure in the state of Kentucky
- At least one year of experience working as an LPN
- CPR certification
- Valid driver's license and reliable transportation
