We are seeking a highly skilled Primary Care Physician to join our team at Valor Healthcare, Inc. as a key member of our Community Based Outpatient Clinic (CBOC) in Cookeville, TN.
Key Responsibilities- Provide high-quality medical treatment and personal care services to patients with diseases and injuries.
- Collaborate with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) and expanded PACT Team to develop patient care plans.
- Actively diagnose and treat patients, examining them for symptoms of organic or congenital disorders, determining the nature and extent of injury/disorder, and performing comprehensive physical examinations.
- Develop and implement patient management plans, assist in provision of continuity of care, order and perform diagnostic tests, prescribe medication, and recommend dietary and activity programs as indicated by diagnosis.
- Counsel patients on the use of prescription medications, educate patients, assess mental health issues, and provide routine health maintenance.
- Evaluate patients' records from medical providers outside the VA and work with these patients utilizing rules set forth by the VA for co-managed care.
- Complete clinical reminders, documentation of the medical record, and solicit medical management consult or input when appropriate.
- 4+ years of clinical medical practice experience, with a government healthcare environment preferred.
- Doctor of Medicine or Osteopathic Medicine Degree.
- Board certified in family practice or internal medicine.
- Credentialed and in good standing through the Veterans Health Administration (VA).
- Valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
- Current certification in Basic Life Support and ACLS, as specified by individual VAMC contract.
- Current, unrestricted Drug Enforcement Administration (DEA) registration.
- Proficiency in written and spoken English.
- Excellent computer skills.
- Energetic and optimistic demeanor.
- Strong service mentality and a focus on achieving all aspects of defined service standard.
