We are seeking a skilled Licensed Practical Nurse (LPN) to join our Primary Care team at Ochsner Health System. As an LPN, you will play a vital role in providing high-quality patient care and working collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities:

  • Provide direct care to assigned patients, including administering medications and performing various nursing procedures.
  • Work closely with registered nurses and other healthcare professionals to develop and implement individualized patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to a culture of safety and excellence within our organization.

Requirements:

  • Current LPN licensure in the state of practice.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • IV therapy credentials (preferred).
  • Excellent organizational and time management skills, with the ability to work in a fast-paced environment.
  • Strong interpersonal and delegation skills, with the ability to work effectively with patients, families, and healthcare team members.
