About the Role

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.

Key Responsibilities
  • Develop and implement individualized physical therapy plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify areas of improvement and set realistic goals.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care and communication.
  • Provide education and training to patients and caregivers on proper techniques and strategies for optimal recovery.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current and unrestricted Physical Therapy license.
  • Minimum of one year of experience in home health or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
