Job Summary

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Medi Home Hospice. As a key member of our interdisciplinary team, you will provide essential social work services to patients and their families, ensuring their emotional and practical needs are met during this challenging time.

Key Responsibilities:

  • Provide social work services in accordance with the physician's plan of treatment, working closely with the interdisciplinary team to ensure comprehensive care.
  • Assess patients' and families' needs, providing bereavement counseling and support as needed.
  • Serve as a liaison with community resources, connecting patients and families with essential services.
  • Collaborate with the team to develop and implement individualized care plans, ensuring patients' and families' needs are met.

Requirements:

  • Master's or Bachelor's degree in social work from a Council of Social Work Education-accredited or approved program.
  • Current licensure to practice as a social worker in the state of VA.
  • Minimum one year of Hospice experience preferred.
  • Valid Driver's License.
