About the Role

We are seeking a highly skilled and compassionate Nephrology Transplant Care Specialist to join our team at Henrico Doctors' Hospital Forest. This is an exciting opportunity to work in a dynamic environment where you can make a real impact on the lives of our patients.

Job Summary

In this role, you will be responsible for coordinating and facilitating the pre and peri-operative processes upon notification of organ availability for a specific patient. You must be able to assess transplant patients for specific needs as well as educate the patient and their family regarding their specific needs related to the kidney transplant, including the ongoing follow-up care requirements.

  • You will be responsible for planning follow-up care in collaboration with the referring physician, transplant surgeon, and other care providers.
  • You will analyze pertinent data, communicate findings, document information, and assume accountability for decisions and actions.

Requirements

To be successful in this role, you must have:

  • An active RN license in the state of Virginia.
  • A BSN preferred.
  • Three years of nursing experience, with 1 year in ICU or one year case management preferred.
