We are seeking a skilled Case Management Specialist to join our team in Brooklyn, New York. This is a full-time position that requires a strong understanding of case management principles and practices.

Responsibilities
  • Conduct initial intakes and assessments of tenants and their families
  • Develop and implement individualized plans to achieve stability and self-sufficiency
  • Work closely with tenants to identify and access community resources
  • Maintain accurate records and reports
  • Participate in case conferences and provide updates on tenant progress
Requirements
  • Bachelor's degree in social work or a related field
  • Experience working with vulnerable populations, particularly those struggling with mental health issues or substance abuse
  • Strong case management skills and knowledge of relevant laws and regulations
  • Ability to maintain confidentiality and handle sensitive information
  • Excellent communication and interpersonal skills
