Job Summary

The Patient Care Specialist will be responsible for providing exceptional patient care and support in our hemodialysis clinic. You will work closely with our experienced nursing staff and other care team members to deliver high-quality care and services to our patients.

Responsibilities

Your key responsibilities will include:

  • Providing direct patient care and support, including vital sign monitoring, medication administration, and blood pressure management.
  • Assisting with dialysis treatments, including setting up equipment, performing vascular access procedures, and maintaining accurate records.
  • Developing and implementing individualized patient care plans that address their unique needs and goals.
  • Collaborating with healthcare professionals to coordinate patient care and ensure seamless transitions between settings.
  • Maintaining accurate and timely documentation of patient information and care provided.

Requirements

To be successful in this role, you will need:

  • A minimum of 1 year of experience in a hemodialysis or acute care setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • Able to work effectively in a fast-paced, dynamic environment with minimal supervision.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
  • Basic computer skills and proficiency in electronic medical records systems.
