Hospice Nurse

As a Hospice RN, you will be responsible for assessing, delegating, coordinating, and evaluating the nursing care provided to our patients. You will ensure the provision of quality care through the utilization of the nursing process, established standards of care, policies, and procedures.
You will initiate the Plan of Care and necessary revisions, and be responsible for the overall care of the patient. You will also initiate appropriate measures to promote symptom management and patient-driven care for terminally ill patients.
The Hospice RN is directly responsible to the Director of Hospice. This is a day shift position, Monday through Friday, 8am-4:30pm, with every 4th weekend.
Key Responsibilities:
- Assess and evaluate patient needs
- Develop and implement individualized care plans
- Coordinate and delegate care to other healthcare professionals
- Monitor and evaluate patient outcomes
Requirements:
- Current active state license
- Minimum 2 years of experience
- 40 hours guaranteed
