Job Title: Clinical Care Coordinator

Estimated Salary: $90,000 - $115,000 per year

This is a dynamic role for a motivated nurse who wants to make a difference in patient care. As a Clinical Care Coordinator, you will work closely with our interdisciplinary team to develop and implement individualized care plans.

About the Role:

We are seeking a highly skilled registered nurse to join our team as a Clinical Care Coordinator. This is a full-time position working 72 hours per pay period, including weekends and holidays. The ideal candidate will have a strong background in nursing and excellent organizational skills.

Responsibilities:

  • Conducts initial assessments to identify patient needs.
  • Develops and implements individualized care plans.
  • Maintains accurate records and reports.
  • Collaborates with other healthcare professionals to ensure seamless care transitions.
  • Stays current with best practices and regulatory requirements.

Qualifications:

The ideal candidate will have:

  • A graduate degree in nursing.
  • A current State of Michigan Registered Nurse license.
  • BLS certification from American Heart Association (AHA), American Red Cross (ARC), or Canadian equivalent.
  • Experience in critical care or cardiology preferred.
