About the Job

Ontario Center is seeking a skilled LPN Supervisor to join our team. As a key member of our healthcare team, you will be responsible for conducting facility rounds, monitoring assignments of aides, and supervising medication assistance times. Your expertise will ensure the highest quality of care and quality of life for our residents.

Key Responsibilities

Conduct facility rounds to ensure the well-being of our residents
Monitor assignments of aides to ensure proper work flow
Supervise medication assistance times to ensure proper recording of assistance and refusals
Intervene in resident conflicts, outbursts, and behavior issues
Facilitate communication to ensure medical/mental health situations are addressed timely
Ensure all aspects of facility incident/accidents are completed and reported to the EHP Coordinator/Administrator

Requirements

Must be able to work as a team member
Valid state LPN license
Must be in good standing with State Registry
