About the RoleWe are seeking a skilled Field-Based Healthcare Specialist to join our team. This role requires traveling up to 80% of the time to patients' homes, ensuring quality care and improving patient outcomes.
Responsibilities- Perform in-home visits with patients to assess their needs and provide quality care.
- Review data collected to identify gaps in care and implement corrective measures.
- Build positive relationships with patients and their families through effective communication and collaboration.
- A high school diploma or equivalent is required.
- The candidate must have a reliable vehicle for travel and hold a valid driver's license.
- Medical Assistant (CMA) Certification or Certified Nurse Assistant (CNA) Certification is preferred.
- Current CPR certification is mandatory.
- At least two years of experience in a healthcare environment, preferably in a field-based setting.
- Familiarity with medical terminology and ability to perform point-of-care testing.
