About the Role

We are seeking a skilled Field-Based Healthcare Specialist to join our team. This role requires traveling up to 80% of the time to patients' homes, ensuring quality care and improving patient outcomes.

Responsibilities
  • Perform in-home visits with patients to assess their needs and provide quality care.
  • Review data collected to identify gaps in care and implement corrective measures.
  • Build positive relationships with patients and their families through effective communication and collaboration.
Requirements
  • A high school diploma or equivalent is required.
  • The candidate must have a reliable vehicle for travel and hold a valid driver's license.
  • Medical Assistant (CMA) Certification or Certified Nurse Assistant (CNA) Certification is preferred.
  • Current CPR certification is mandatory.
  • At least two years of experience in a healthcare environment, preferably in a field-based setting.
  • Familiarity with medical terminology and ability to perform point-of-care testing.
