Key Responsibilities
- Provide personal care based on assigned patient needs and respond promptly to requests.
- Immediately report any inability to perform tasks to the Case Manager.
- Utilize effective communication techniques with patients and families, ensuring confidentiality and respect.
- Ensure patient safety by using equipment properly (e.g., foot stools, side rails, O2).
- Deliver personal care services, including bathing, oral hygiene, and changing bed linens as required.
- Assist patients with dressing and undressing as needed.
- Plan and prepare nutritious meals, including shopping, as assigned.
- Assist with feeding patients as directed.
- Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure, as ordered.
- Observe and care for patients' skin to prevent tissue breakdown.
- Report significant changes in patient condition to the Case Manager.
- Be aware of interpersonal issues involving caregivers or others living with the patient.
- Support patients in ambulation and exercise as instructed by healthcare professionals.
- Assist with self-administered medications as permitted by state regulations.
- Provide assistance with bedpans and urinals as needed.
- Help with light laundry tasks as assigned.
- Perform range of motion exercises and other simple procedures as directed.
- Offer respite care for patients and their families when on-site.
- Maintain cleanliness and orderliness in the patient's living area as assigned.
- Adhere to documentation and care standards set by the organization.
- Participate in quality improvement initiatives and activities.
- Respect the environment and personal needs of patients and their families.
Qualifications
- Meets state training requirements.
- Completed a minimum of 75 hours of classroom and supervised practical training, with 16 hours of classroom training prior to practical training.
- At least six months of experience in providing personal care.
- Must be at least 18 years old.
- Ability to read, follow written instructions, and document care provided.
- Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
- Positive references from previous employers.
- Ability to work independently with minimal supervision.
- Comfortable with issues surrounding death and dying.
- Exhibit a calm demeanor in patients' homes.
- Current CPR certification is required.
- Demonstrate patience, tact, and good personal hygiene.
- Successfully complete a probationary period to ensure competency in required qualifications.
- Strong oral and written communication skills.
- Good organizational abilities.
