Key Responsibilities:
- Case Management Participation: Engage in all aspects of the CMP, ensuring adherence to case management standards.
- Nursing Expertise: Provide in-depth knowledge of the case management process, including assessment, planning, implementation, coordination, and monitoring.
- Strategy Development: Create and execute local strategies for inpatient, outpatient, onsite, and telephonic case management.
- Tool Implementation: Develop and utilize tools to enhance case management, including patient identification and assessment tools, clinical guidelines, and CM software.
- Collaboration: Work closely with community agencies and organizations to maintain effective communication and resource sharing.
- Data Management: Collect and document patient care data accurately to support treatment planning.
- Treatment Planning: Formulate comprehensive treatment plans that encompass preventive, therapeutic, rehabilitative, and psychosocial interventions.
- Patient Education: Provide essential healthcare instructions to patients and caregivers based on identified needs.
Additional Responsibilities:
- Technology Utilization: Leverage automated programs and IT tools for proactive patient management and engagement.
- Multidisciplinary Coordination: Assist in coordinating a multidisciplinary team to address the healthcare needs of patients.
- Policy Development: Create and implement policies for home health assessments and outcome measures.
- Reporting: Prepare routine reports and conduct analyses to evaluate case management effectiveness.
- Regulatory Compliance: Ensure adherence to relevant regulatory requirements and apply medical care criteria.
- Resource Management: Provide insights on resource utilization and make recommendations for optimization.
- Patient Follow-Up: Establish mechanisms for effective post-discharge follow-up in various healthcare settings.
- Professional Development: Plan for ongoing professional growth and development in nursing practice.
Qualifications:
- Education: Bachelor's degree in Nursing from an accredited institution; a Master's degree in Nurse Case Management is preferred.
- Experience: Minimum of 5 years of experience as a Registered Nurse Case Manager within the last 7 years.
- Certification: Current certification from recognized organizations in case management.
- Licensure: Active and unrestricted Registered Nurse license.
- Security Clearance: Ability to pass a government background check.
- Life Support Certification: Current BLS Healthcare Provider certification required.
