Chinatown Service Center seeks a skilled Registered Nurse to fill the role of Patient Care Coordinator. The ideal candidate will have excellent communication skills and be able to work effectively with diverse populations.
Key Responsibilities:- Perform Annual Wellness Visits for Medicare patients, providing thorough assessments and developing personalized care plans.
- Coordinate care for patients enrolled in Chronic Care Management, ensuring seamless transitions and optimal health outcomes.
- Provide Transitional Care Management for patients post-discharge, facilitating smooth reintegration into their communities.
- Collaborate with multidisciplinary teams to create Health Action Plans (HAPs) tailored to individual patient needs.
- Evaluate patient progress, making adjustments to HAPs as necessary to ensure goals are met.
- Graduate of an accredited school of nursing with current California RN licensure.
- BLS certification required; Bilingual English and Chinese/Spanish proficiency preferred.
- Strong critical thinking abilities, effective communication skills, and basic computer literacy.
- Able to work independently, prioritize tasks, and maintain accurate records.
We offer a competitive salary range: $ 100,000 - 110,000 per year, depending on experience and qualifications.
