Key Responsibilities- Collaborate with the healthcare team to develop, implement, and revise the case management program, ensuring seamless coordination of patient care.
- Act as a liaison between patients, families, nurses, physicians, and other healthcare professionals to optimize patient outcomes.
- Coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge, ensuring efficient progression through the hospitalization process.
- Ensure regulatory compliance by managing data required for accreditation and regulatory purposes.
- Perform other job-related duties as assigned by management.
- Bachelor's Degree in Nursing (BSN): Preferred
- Experience in a care management role: Preferred
- Registered Nurse (RN) licensure in the state of practice: Required
- Case management certification: Preferred
