Key Responsibilities- Assess and develop individualized care plans for residents, ensuring their physical, emotional, and social needs are met.
- Collaborate with the healthcare team to coordinate care services, including medication management, wound care, and rehabilitation.
- Provide education and support to residents and their families on health-related topics, promoting independence and well-being.
- Conduct regular assessments and evaluations to ensure residents' needs are being met, and make adjustments to care plans as necessary.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
- Current CPR certification.
- Ability to communicate effectively with diverse populations, both verbally and in writing.
- Leadership skills and ability to motivate and direct other personnel.
- Basic computer literacy and familiarity with electronic medical records (EMR) systems.
This role requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The successful candidate must be able to lift up to 25 pounds, push, pull, and move residents as needed. Regular attendance and punctuality are essential.
