Responsibilities:
Are you dedicated to providing exceptional patient care? Synchrony Rehab, a division of Synchrony Health Services, is in search of a licensed Physical Therapist. In this role, you will be responsible for delivering comprehensive physical therapy services, including evaluation, treatment planning, and therapeutic interventions within a collaborative healthcare setting. Your expertise will contribute to the development of quality care programs and enhance patient outcomes.
Key Duties:
• Deliver therapy services, including initial assessments and tailored treatment plans across various settings.
• Collaborate with interdisciplinary teams to create and implement effective care plans.
• Monitor and assess patient progress, providing valuable feedback to the healthcare team.
• Maintain accurate documentation of patient care and treatment efficacy.
• Educate patients and their families on rehabilitation processes and resources for continued independence.
• Serve as a knowledgeable resource for staff training and development in physical therapy practices.
• Assist in the formulation of student training programs as needed.
• Ensure the delivery of cost-effective care while adhering to ethical standards.
• Report any resident concerns to the appropriate supervisory personnel.
• Travel to various locations as required for patient care.
• Supervise physical therapy assistants in compliance with state regulations.
• Uphold resident rights and maintain professional competencies.
• Participate in organizational survey preparations and other assigned tasks.
Qualifications:
Education: Bachelor’s Degree in Physical Therapy
Experience: 0-1 years in a relevant field
Licenses and Certifications: Must possess a valid state license for physical therapy. Current CPR certification preferred.
Physical Requirements:
Ability to perform physical tasks including lifting, standing, and walking throughout the workday. Must be capable of lifting a minimum of 50 lbs and maintaining effective communication with colleagues and patients.
