Job Summary

As a Licensed Practical Nurse (LPN) at Altru Health, you will play a vital role in delivering high-quality patient care. Your primary responsibility will be to perform and document assessments, ensuring the health history and admission process is completed. You will identify discharge planning and education needs, provide patient care, and perform personal cares. Effective communication with patients and the multidisciplinary team is essential to ensure quality and continuity of care.

Key Responsibilities
  • Assist registered nurses and other licensed practitioners in identifying patient needs, priorities of care, and goals.
  • Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
  • Modify the plan of care in collaboration with licensed practitioners.
  • Perform other duties as assigned to meet the needs of the department/organization.
  • Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
  • Assign or delegate the responsibility for performance of nursing interventions to other persons for which that person has the necessary skills and competence to accomplish safely.
  • Identify the early warning signs of a change in a patient's condition and report appropriately.
  • Implement nursing interventions and prescribed medical regimens in a timely and safe manner and document nursing care provided accurately and timely.
  • Collaborate and communicate relevant and timely patient information with patients/families and other multidisciplinary team members to ensure quality and continuity of care.
  • Report the patient's self-report/symptoms of pain promptly according to standard operating procedure.
  • Participate in the health education approved by a licensed practitioner.
  • Assist in the discharge planning, appropriate education, and counseling based on identified needs to optimize patient/family understanding.
Certifications and Requirements
  • Licensed Practical Nurse - Minnesota Board of Nursing
  • Basic Life Support (BLS) - American Heart Association
Working Conditions

This position requires frequent standing, walking, and reaching. The ability to lift up to 50 pounds and push/pull up to 25 pounds is necessary. The work environment is primarily indoor with occasional outdoor exposure. The position requires the ability to work in a fast-paced environment and prioritize tasks effectively.

Driving Requirement

This position is classified as an Infrequent Driver.
