We are seeking a seasoned Registered Nurse/Patient Care Manager to lead our home health team in providing compassionate and personalized care to patients. Salary:

$80,000-$90,000 per year, based on experience

Job Description:

As a Registered Nurse/Patient Care Manager, you will be responsible for coordinating patient care activities, managing staff performance, and collaborating with interdisciplinary teams to develop effective care plans. Your expertise in home health nursing will ensure that our patients receive the highest level of care and support in the comfort of their own homes.

Required Skills and Qualifications:

To succeed in this role, you will need:

  • Current RN licensure in the state where you practice
  • A minimum of 2 years of experience in home health care settings, preferably in a management or supervisory capacity
  • Knowledge of regulatory requirements, including Medicare Conditions of Participation and federal, state, and local laws
