About the Role
This Clinical Pharmacist position is responsible for providing expert medication therapy management services to patients across various settings. You will be working closely with healthcare providers to optimize patient treatment plans, ensure medication safety, and promote better health outcomes. Key Responsibilities
Your main duties as a Clinical Pharmacist will include:
- Gathering comprehensive medication histories and reconciling medication lists as needed.
- Conducting thorough medication reviews to identify opportunities for improvement and making evidence-based recommendations.
- Identifying and reporting suspected adverse drug reactions, adherence gaps, and barriers to patient adherence.
- Collaborating with healthcare providers to address concerns related to medication use, including allergies, medical conditions, drug interactions, dosing, and administration methods.
- Accurately documenting all clinical activities and interventions in electronic records.
- Serving as a consultation resource for medical professionals and responding to inquiries about drug therapy and products.
- Providing education and guidance to patients and caregivers on medication-related topics.
Requirements
To succeed in this role, you will need:
- A Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution or equivalent experience.
- A current, unrestricted pharmacist license to practice in the state of New Hampshire, with willingness to pursue reciprocity in multiple states if required.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Experience in medication therapy management, preferably in an outpatient setting.
- Proficiency in electronic health record systems and other relevant software applications.
