We are seeking a skilled Pharmacist - PRN to join our team at PharMerica, a leading provider of pharmacy services to senior living communities, skilled nursing facilities, and post-acute care organizations.
Key Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Collaborate with the healthcare team to provide clinical and technical support, ensuring safe and effective medication management.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
