Case Management Specialist

Work Hours: 8 AM - 5 PM
Compensation: $28/hour
Key Responsibilities:
- Assess the needs of clients, identify available resources, and recommend or facilitate plans for optimal outcomes.
- Assist in the development of comprehensive long-term care plans and identify necessary providers, specialists, and community resources.
- Coordinate effectively between clients, families, and care provider teams to ensure accessibility of services.
- Provide resource support for clients and their families in areas such as employment, housing, independent living, and more based on service assessments.
- Monitor and evaluate care plans and client progress, making recommendations based on identified needs.
- Engage with long-term care healthcare providers to ensure that client needs are met efficiently.
- Document and maintain comprehensive records of client information and care management activities to comply with regulatory standards.
Qualifications:
- Minimum of 2 years of experience in case management.
- Familiarity with electronic medical records.
- Proficient in Microsoft Office Suite.
- Bachelor's Degree in a relevant field.
Additional Requirements:
Must possess reliable transportation for client visits.
Language Proficiency: Bilingual in English and Spanish is required.
