Job Summary:

We are seeking a skilled Clinical Chaplain to provide spiritual support and counseling to home care patients, families, and staff in a culturally inclusive manner. As a member of the interdisciplinary patient care team, you will participate in patient care planning, document interventions, and meet regulatory guidelines.

Key Responsibilities:

  • Administrative Duties:
    • Participate in Quality Assurance Activities, including committees for Patient Satisfaction Scores, QI Projects, and UBT.
    • May serve on committees, such as Bioethics, EAP, LCP, and ELOA.
    • Conduct in-services and CEU programs to improve Integrated Model of Care.
    • Provide staff support to maintain Spiritual Resilience in Healthcare.
  • Direct Patient Care:
    • Provide spiritual support and counseling to home care patients, families, and staff in a culturally competent manner.
    • Participate in creating patient care plans, including acuity mapping, and document interventions and outcomes.
    • Support the admission/consent process and explain hospice benefits to patients and families.
    • Support bereavement care plans for surviving family members.

Requirements:

  • Minimum of one year of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Ambulatory, Home Health, Palliative Care, or Hospice setting.
  • Masters Degree in Divinity, Theology, or related field from an accredited Seminary, College, or University.
  • Driver's License (California) and Basic Life Support required at hire.

Preferred Qualifications:

  • Two or more years of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Home Health, Palliative, or Hospice Setting.
  • Board Certified Chaplain Certification from Association of Professional Chaplains.
  • Minimum of four units of Clinical Pastoral Education from an ACPE accredited program.
