DePaul Community Resources is seeking a dedicated Case Management Support Specialist to join our team in a full-time capacity. This role is primarily remote, with occasional requirements to attend meetings at the office.
Role Summary:
The Case Management Support Specialist is responsible for the meticulous review, updating, and completion of documentation, case notes, and various records within the ID/DD Services Division. This position provides essential support to the Residential Coordinators by ensuring that all case management documentation is accurate and up-to-date. The role does not involve fieldwork, and tasks are delegated by the Residential Manager based on the needs of the team.
Supervision:
This position reports directly to the Regional Manager.
Key Responsibilities:
- Review and assess documentation such as monthly progress notes and case narratives, providing constructive feedback to the assigned Residential Coordinator and Supervisor.
- Conduct thorough reviews of assigned documentation to facilitate quarterly person-centered reviews for submission.
- Draft initial versions of annual plans by incorporating information from assessments and evaluations, forwarding them to the Residential Coordinator for finalization.
- Ensure compliance with deadlines established by the electronic health record system and supervisory guidelines.
- Participate in required training sessions as assigned.
- Identify and report trends or areas of concern in case records to the Residential Coordinator and Supervisor.
Applicants should possess an Associate's degree in a human services field and have at least one year of experience working with the ID/DD population, or two years of equivalent experience. Personal experience may also be considered.
Work Environment:
The work environment is primarily office-based, with some fieldwork required. The role typically involves regular weekday hours, though flexibility may be necessary.

Physical demands include the ability to operate standard office equipment, effective communication, and the capacity to navigate various work environments. Reasonable accommodations may be provided as needed.
