Position Summary:

The Advanced Practice Provider plays a vital role within a multidisciplinary team that includes healthcare professionals, medical assistants, nursing staff, behavioral health specialists, and additional support personnel. This role is integral to fulfilling the mission and values of MCHCC by delivering primary care services to patients. The position may involve after-hours coverage and occasional weekend shifts as necessary.

Key Responsibilities:

  • Deliver primary healthcare services to MCHCC patients
  • Utilize Electronic Medical Records (EMR) for patient management
  • Engage in various communication platforms for staff collaboration
  • Perform additional duties that contribute to the overall objectives of the organization, as assigned
  • This overview does not encompass all tasks associated with the role

Qualifications:

Essential Skills and Competencies:

  • Bilingual proficiency in English and Spanish is preferred
  • Experience with Trauma-Informed Care approaches is advantageous; training will be provided
  • Familiarity with Motivational Interviewing techniques is preferred; training will be provided
  • Exhibit strong interpersonal communication skills
  • Availability to work evenings and weekends as required

Educational Background and Experience:

  • Valid Indiana licensure as a nurse practitioner or physician assistant
  • DEA registration
  • Board certification
  • Collaborative practice agreement with MCHCC physicians
  • BLS certification is mandatory

Physical Requirements:

  • Capability to lift items weighing up to 10 pounds
  • Ability to navigate within the office and patient rooms
  • Capacity to position oneself effectively for patient care
  • Ability to spend extended periods in front of a computer

  • Compliance with federal regulations regarding identity and work eligibility verification is required
