Company Overview: Empath Health is dedicated to providing comprehensive hospice services across multiple counties in Southwest Florida. We are currently seeking a skilled and collaborative RN Triage Case Manager to join our team.
Role Overview: The RN Case Manager is responsible for developing and overseeing a personalized care plan, acting as the primary liaison for patients and their families throughout the hospice journey. This role requires exceptional listening, observational, and communication skills to deliver nursing care in sensitive and emotionally charged environments.
Key Responsibilities:
- Regularly reviews and revises care plans, physician directives, medication profiles, and other pertinent documentation.
- Provides education to patients, family members, and caregivers regarding the illness trajectory, available palliative options, the process of dying, and safety protocols.
- Must be a graduate of an accredited nursing program and possess a valid RN license in Florida.
- BSN or MSN is preferred.
- A minimum of one year of experience in an acute care hospital setting with direct patient interaction; prior experience in hospice care is advantageous.
