End-of-Life Care Specialist

We are seeking a skilled and compassionate End-of-Life Care Specialist to join our team at Hospice Of Hope, Inc. As a key member of our care team, you will provide one-to-one care to terminally ill patients, working closely with families and healthcare providers to ensure the highest level of care and support.
Key Responsibilities- Provide compassionate end-of-life care to patients and their families, assessing and responding to their needs in a timely manner
- Develop and implement individualized care plans, working collaboratively with healthcare teams to ensure seamless care coordination
- Document complex medical information, using clinical tools and data to inform care decisions
- Establish strong relationships with patients, families, and healthcare providers, utilizing effective communication and interpersonal skills
- Graduate of an accredited School of Nursing and current licensure in Kentucky and Ohio
- At least one year of experience in patient care, preferably in a hospital, nursing home, or related healthcare facility
- Personal qualities of empathy, responsibility, and dependability, with effective communication and leadership skills
Our care team provides support to patients in private residences, nursing homes, and assisted living facilities in Kentucky and Ohio. As a member of our team, you will work independently, with occasional supervision, and must be able to travel to client homes and other facilities as needed.
Sign-on Bonus: $2,500 after six months of full-time employment and $2,500 after one year of full-time employment
